Our Services

Dr. Britt provides tailored cognitive care planning for those diagnosed with dementia or mild cognitive impairment. Dr. Britt also provides ongoing support services either by monthly subscription or on an hourly basis. Additional services are available on a case-by-case basis.

Evaluation & CarePlanning

We use the latest validated evaluation tools to build care profiles for clients and caregivers. Standard evaluations may include cognitive screening, behavioral symptom evaluation, depression and anxiety screening, and caregiver stress.

Personalized and comprehensive written Cognitive Care Plan for client, caregiver/SO and family may include lifestyle modifications: medication review, physical exercise, spiritual practices, diet, society activity, limiting alcohol and smoking, managing Diabetes and/or hypertension, sleep, home safety, brain activities, and Advance Care Plan documentation and planning.

Recommend therapy for caregivers and family members in addition to person with MCI/ADRD [speech, occupational, physical therapy]

CarePlan Support

CogWell provides optional ongoing support services for patients and their families after completion of the Evaluation and CarePlan Package. CarePlan Support includes the following:

  • Twice monthly check-in calls or visits to document progress and answer questions

  • Group meetings for caregiver support (via videoconference and/or in person)

  • Access to CogWell preferred provider references for support services, e.g., physical therapists, psychologists, legal counsel, nurse care management, neurologists, psychiatrists, family counseling, in-home care providers, meal preparation, caregiver services, etc.

  • Ongoing education and resource sharing. Future webinars to include video presentations by leading researchers, physicians and dementia specialists

Comprehensive Care Management

Dr. Britt can provide supervision and hands-on administration of ongoing client care. Available by individual agreement.
